Eligibility Criteria (As per Indian Guidelines):
1. Nationality
- Must be a citizen or permanent resident of India.
- Overseas Citizens of India (OCI) engaged in health outreach in India are also eligible.
1. Professional Background
- Open to individuals from the following backgrounds:
- Medical and Allied Health Professionals (MBBS, BAMS, BHMS, BPT, Nursing, etc.)
- Public Health Experts (MPH, MD-Community Medicine, etc.)
- NGO Leaders or Health Workers involved in community-based healthcare
- Social Workers or Educators with documented contribution to health awareness or outreach
- Government or Private Sector Professionals engaged in health service delivery
3. Experience and Contributions
- Minimum of 2 years of documented experience in:
- Community health outreach initiatives
- Public health education or awareness programs
- Rural health service delivery
- Disease prevention campaigns (e.g., tuberculosis, malaria, HIV/AIDS, NCDs)
- Health camps or mobile clinics for underserved populations
- Contributions must align with Indian public health priorities, such as:
- National Health Mission (NHM)
- Ayushman Bharat
- Immunization drives
- Maternal & child health
- Mental health awareness
4. Impact Evidence
- Should provide evidence of measurable impact, such as:
- Number of beneficiaries
- Collaborations with local bodies/NGOs
- Press/media mentions or government recognition
- Testimonials from beneficiaries or supervising authorities
5. Ethical and Legal Standing
- Must have a clean record with no pending legal or disciplinary actions.
- Must adhere to the Medical Council of India/NMC or respective council guidelines (if registered).
6. Age Limit
- No strict age limit, but applicants below 21 years must demonstrate exceptional achievements.
7. Nomination or Direct Application
- Applicants can self-apply or be nominated by:
- Institutions
- Government bodies
- NGOs or professional societies

Original certificate must be reported as lost, damaged, or irreversibly altered.

A signed declaration or visual proof (e.g., photo of damaged certificate) may be required.

Reissue is permitted for minor corrections (e.g., spelling errors in name, designation, suffix).

Supporting government-issued ID or relevant documents must be submitted.

Upgrade from Provisional to Full Honor:

Participants who received a Provisional Fellowship Honor and later completed the full ceremony requirement are eligible for a reissue with the gold seal.

If the original certificate was never received due to verified courier issues, a free reissue may be approved once.
